To prove that you live in Ontario, you’ll have to give your area DSO a copy of one of these documents: your health card, lease or rental agreement, statement of direct deposit for Ontario Disability Support Program (ODSP), a bank account statement with your address on it, driver’s license, or other government-issued photo ID.
